Abstract

There is a review of the mechanisms of pancreatic beta-cell programmed death (apoptosis) in type 1 and 2 diabetes mellitus. Based on the evidence of anti-apoptotic physiological effects of insulin the necessity of new therapeutic strategy focused on maximal beta-cell function and mass preserving in diabetic patients using modern effective insulin therapy regimen is ground. The advantages of the both long- and short-acting insulin analogues (Lantus® and Apidra®) for early aggressive and safe glucose lowering therapy warranted to beta-cell preservation are argued.
