Abstract

Introduction. Clinical course of diabetes mellitus is often adhered by purulent-septic pathology, and endogenous
intoxication syndrome (DMSEI) develops. Disorders of the compensatory mechanisms of homeostasis
providing — volume- and osmoregulatory function of the kidneys arise. Preparations of five-atomic alcohol
(xylate) and angiotensin-converting enzyme inhibitor (ACEI, lizinopril) meet the correction requirements.
The purpose of the work. To investigate the effect of xylate in combination with angiotensin — converting
enzyme inhibitor on the volume- and osmoregulatory function of the kidney in patients with DM type 2.
Materials and Methods. The groups under study consisted of 31 patients (group I) with DMSEI who received
proper therapy, including. xylate (3 ml/kg/h i.v, daily) and 22 patients (gr. II: APs ≥ 140 mm Hg. and/or
APd ≥ 90 mmHg, lizinopril 10 mg/day, during 7 days).
Results. In patients of group II-A sodium clearance was higher 63%, standardized excretion — 68 %, excretion
fraction — 72 % (p < 0.05) than in patients of gr. I-A. The corresponding indicators of the osmoregulatory
function — clearance, standardized excretion and excretory fraction of osmotic active substances (OAS) were:
12 %, 13 %, 18 % (p < 0.05). Xylate against a background of ACEI pharmacodynamics increased sodium clearance,
exceeding this indicator in the group of comparison 58 %, and OAS clearance predominated index data
39 % without ACEI use.
Conclusions. ACEI in combination with xylate interact with the volume and osmoregulatory functions of
the kidneys in patients with DM type 2, complicated by endogenous intoxication syndrome.
